Opening Discussion

c. Explain the purpose of the medication management consultation:
"In this pharmacy, we keep records about you and the medications you take to help ensure that you get the most benefit from them. I'd like to take a few minutes to ask you some qu stions."
d. Ensure confidentiality:
"Of course, any information you give me will be strictly confidential and will be kept in the computer in your personal patient record."
e. Get consent:
"Would it be all right for me to continue with this now?"

Inquiry of Personal Information
a. Introduce the section:
"First I need some information for our records about yourself."
b. Ask personal questions:
"Sometimes your age or what you do for a living can affect your health and medication use so I need to ask you about these."
"What is your birth date?"
"Are you working at the moment?. . . Do you mind telling me what you do?"
Some comments following the patient's responses would help set conversational tone, e.g., "That’s the same month as my birthday ... That sounds like an interesting job."

Discussion of Medical Conditions and Medication Use
a. Introduce the section:
"Now I'd like to ask about your medical conditions at,d medication use."
b. Ask about the condition currently being treated:
"First, what brought you to the doctor?"
c. About existing conditions in general:
"Do you have any medical conditions at the moment, such as diabetes, high blood pressure, heart condition, ulcer, and so on?"
d. Gather information about each condition and the medications associated with it before proceeding to the next condition.
• Ask about the duration of the condition:
"Now I'd like to discuss each of your conditions. First, how long have you had diabetes?"
• Ask about medication used for the condition:
"Are you taking any medication at the moment for your diabetes?"
• Ask details about each medication sequential]) .

• Probe for details, if necessary, to ascertain adherence:
